•	Call our laboratory: When you contact our laboratory you will speak with a genetic counselor. Her role is to provide information about the DNA test you are interested in and to answer your questions. She will gather information about your current symptoms and/or your family history. All information from you is confidential. The genetic counselor will send you the appropriate sample collection kit for the test of interest to you. To reach one of our genetic counselors, please call 800-320-1807, Monday through Friday, between 8:30 a.m. and 5:00 p.m. Mountain Time or email us by filling out the form in the "Contact Us" section of our web site.
•	Collect and submit your sample: For all tests, the name, address, phone and fax number of a physician are required in order to report your test results. Samples submitted without a physician indicated on the Test Request form cannot be processed. To submit a cheek cell sample: We will send you a collection kit that contains cheek brushes, instructions for cheek cell collection and mailing, a Test Request form, a consent form, and instructions for payment for the test. To submit a blood sample: We will send you a collection kit that contains blood tubes, instructions for blood collection and mailing, a Test Request form, a consent form, and instructions for payment for the test.
•	Include payment: Payment by check or credit card for the DNA test is required at the time the sample is sent to us. Please contact Kimball Genetics for information about the cost of the test. A statement indicating that you have paid for the test will be sent to you. You can submit this to your insurance company for reimbursement.
•	Receive your test results: We will report your test results to the physician indicated on your Test Request form and mail a copy of your report to you. If you are a resident of New York state, you can obtain a copy of your test report from your physician. The report contains useful information about the interpretation of your test results and their relevance to family members.
